We can complete a full cognitive assessment and risk factor assessment to determine your risk of dementia and other cognitive impairments. this may include speech evaluations, neurology evaluations, laboratory testing, and electrodiagnostic testing. 

Our team will assist with managing medications that can help treat and prevent cognitive impairments. We can evaluate for candidacy for medications that can slow dementia progression. 

We can assist you with keeping you independent as long as possible. This will include fall assessment testing, driving evaluations, activities of daily living, medication management and gait training. 

We work with your family and caregivers to prepare for the future. We meet to discuss how you are doing from both a cognitive and functional status. We can prepare your family for the needs you may have in your home in the near or long term future. 

We will help you have those difficult conversations with you and your families. We can assit with coming up with a plan for when you cannot make those decisions for yourself. This may include goals of care discussion, advanced directives, and estate planning. 

We can help decide when palliative care or hospice may be the right choice.
